Is Connect360 free to use?

Yes, Connect360 is completely free for individuals seeking services. There are no fees to create an account, search for services, or connect with care coordinators. Some individual service providers may have their own costs, which will always be disclosed upfront.

What services can I find on Connect360?

Connect360 provides access to a wide range of services including healthcare, mental health support, housing assistance, food programs, employment services, education resources, transportation, childcare, and emergency assistance.

How is my personal information protected?

We use enterprise-grade security measures including encryption, secure data storage, and strict access controls. Your information is protected according to HIPAA standards where applicable. We never share your information without your explicit consent, except as required by law.

Can I update or delete my information?

Yes, Connect360 provides access to services across multiple locations. When searching, you can specify your preferred location or search statewide. Some services may be available virtually regardless of your location.

Can someone else access emergency resources on my behalf?

Yes, family members, friends, or caregivers can access emergency resources for someone in need. However, for ongoing care coordination, the individual receiving services should create their own account when possible.

Care coordinators are trained professionals who help you navigate services, schedule appointments, communicate with providers, track your progress, and ensure you receive comprehensive support. They serve as your personal guide through the care system.

How am I matched with a care coordinator?

After completing your profile, we match you with a care coordinator based on your needs, location, language preferences, and the coordinator's expertise. You can request a different coordinator if the match is not suitable.

How often will I hear from my care team?

Communication frequency depends on your needs and preferences. Typically, you will have an initial consultation followed by regular check-ins (weekly, bi-weekly, or monthly). You can always reach out to your care team between scheduled contacts.

Can I change my care coordinator?

Yes, if you feel your current care coordinator is not the right fit, you can request a change at any time. Contact our support team, and we will help transition you to a new coordinator.
